Hint: Backward bug

This cable and connector were discovered on a job where someone tried to isolate the equipment grounding conductor. If you look close, you'll see the insulation covering on the light green cable is continuous right on through the connector bolt head. The "bug" is even attached backward. This connector (bug) is meant to mechanically fuse the two wires together, with insulation on one of the wires that is highly unlikely.


What's Wrong Here?

The question is what isn't wrong. There were never enough supports on the pipe. There is a coupling missing leaving the conductors inside unprotected.
